If you need any of the following forms completed by your child’s teacher or other school staff, please channel these forms through Lori Vendemia, Learning Resources.

              Teacher Observation Checklists

              Physician Checklists – Psychiatric Checklists

              Private Tutoring Questionnaires

              Teacher Documentation for Psycho-Educational Testing

The forms will be distributed to the appropriate teacher to complete.  Please sign a form for release of information exchange with the receiving agency, physician, etc. so that we may send the information directly to that agency, physician, etc.  Please bring a copy of this form so that we can keep it on file.   This will expedite the process and allow agency, physician, etc. to obtain information from us if needed.
